The Institute for Health Policy continues to provide technical assistance and support to health plan care and disease management programs. This includes clinical case management education for health plan case managers addressing conditions prevalent among the Medicaid population. Sessions may be offered via web modules or one-day seminars and include disease pathophysiology, pharmacotherapy, and patient education and self-management strategies.
