The Institute for Health Policy continues to provide technical assistance and support to health plan care and disease management programs. This includes clinical case management education for health plan case managers addressing conditions prevalent among the Medicaid population. Sessions may be offered via web modules or one-day seminars and include disease pathophysiology, pharmacotherapy, and patient education and self-management strategies.
The Institute for Health Policy, in collaboration with Michigan Medicaid Services Administration, holds Case Management Conferences each year in March and April. These day-long conferences bring together leading content experts to share valuable information, exchange ideas, and encourage innovation in the case management field. Evidence-based initiatives and best practices are shared to improve patient outcomes. The intended audience is Medicaid Health Plan Case Managers, as well as CSHCS Case managers as the topics change each year.
